Copy of current U.S. government-issued identification with photo and signature
Copy of current CPR card
Statement of Proficiency form signed by the physician, Nurse Practitioner or Physician's Assitant if you are working or signed by your instructor if a graduate and (omit if) applying to sit for the clinical exam
Proof of employment (most current pay stub) or certificate of completion if a graduate (how you are applying)
